This anxiety tracker template is an effective way to mood changes, panic attacks, and other emotional symptoms/physical symptoms. Using anxiety tracker worksheets can help you notice possible triggers and track your progress made with techniques such as EFT tapping, cognitive behavioral therapy, etc.

As always, I recommend using a high quality paper (not whatever cheap stuff is in the copier at work) for a better journaling experience. I use HP 32# premium bright white paper. It’s truly a joy to write on.

Theses printable can be used in a specific anxiety journal or as part of a bullet journal. You could even use the PDF file in your favorite digital planner or note taking app! (Warning: tracking my symptoms made me realize I needed to give up coffee. Gak. I love coffee. But it was keeping my anxiety and insomnia ramped up.)
Let’s take a look at the two different anxiety tracker printables. All of the pages are available for download lower down in the post. Look for the row of purple arrows.
The first sheet has more space to write and is perfect for tracking your success with CBT, EFT, reframing, etc. It’s a chart that allows you to record specific situations that bring you anxiety, symptoms, and rate the severity of the episode. There is also space to record what action you took and re-evaluate your self on a 1-10 scale after your intervention. This printable is available in minimalistic black & white and a cuter, more colorful version with self-care clip art:

The second page is a printable symptom tracker with various different common anxious and stressful events and feelings. You can record your symptoms and use color coding to rate the severity of the symptom.
Some stress and anxiety trackers have pre-filled stressors, triggers, or symptoms (for example, having trouble falling or staying asleep, being late to work, or crying). Because stressors and symptoms are personal and unique, I’ve opted to leave the symptoms and stressors column blank. You can fill in each row with whatever is most relevant to you. Here’s a look:
